for problems with a microfilm camera to your PC and to fill in the form before contacting your dealer.

Microfilm readers, microfilm processors and other film equipment can also be filled in the camera form. The fields not applicable to your device can be left empty.
For hybrid systems please decide whether you have a more camera specific or scanner specific problem and select the suitable form.
The form is created under MS Word 97 (*.doc). It can be edited directly on your PC, stored and printed afterwards. This requires Word version 97 or higher installed on your PC.
